Similar rules apply - ALL patient details must be correct, all clinicians must be documented and signed, all signatures must be 'wet'.


Sections 3 and 4 are not strict decision making tools, but should be taken into account when making decisions on behalf of your patient.


Whether you patient has capacity or not, the correct corresponding box must be ticked in section 6.


If a review date is supplied it must be adhered to.
