For a DNACPR form to be valid:
It must contain all patient details: name, address, date of birth, NHS number as a minimum
It must be within its' review date (if supplied) - if not date is written, assume validity for life
It must have a 'wet' signature - photocopied forms are accepted, the signature must be re-signed
It must have all details (name, reg number, grade) of all clinicians creating or altering the form
It does not have to be in colour - many GP practises do not have colour printers
If any information is incorrect or in doubt - assume it is not valid and START CPR
ReSPECT forms
Similar rules apply - ALL patient details must be correct, all clinicians must be documented and signed, all signatures must be 'wet'.
Sections 3 and 4 are not strict decision making tools, but should be taken into account when making decisions on behalf of your patient.
Whether you patient has capacity or not, the correct corresponding box must be ticked in section 6.
If a review date is supplied it must be adhered to.
